Indications
  • Approved Indications
    • Ophthalmic Use (Artificial Tears / Lubricants)
      • Relief of dry eye symptoms such as irritation, burning, and foreign body sensation
      • Management of mild to moderate keratoconjunctivitis sicca
    • Ocular Surface Protection
      • Temporary relief from exposure-related dryness (air conditioning, prolonged screen time, contact lens use)
  • Off-label / Clinically Accepted Uses
    • Lubrication for post-surgical ocular surface recovery
    • Adjunct therapy in certain inflammatory or degenerative ocular surface disorders
Dosage & Administration
  • Adults and Children
    • Instill 1–2 drops in the affected eye(s) 3–6 times daily, or as needed for symptom relief
  • Elderly
    • Same dosing; monitor for adherence and ability to self-administer
  • Special Populations
    • No dosage adjustment required for mild renal or hepatic impairment
  • Administration Notes
    • Wash hands before use
    • Avoid touching the tip of the dropper to the eye or any surface to prevent contamination
    • Remove contact lenses prior to instillation; may reinsert after 15 minutes
Mechanism of Action (MOA)

Hydroxyethyl cellulose is a hydrophilic polymer that increases tear viscosity, enhancing ocular surface lubrication and retention time of tears. Glycerin acts as a humectant, drawing water into the corneal epithelium and maintaining moisture. Together, they form a protective film on the eye surface, reducing friction, minimizing irritation, and promoting epithelial healing.

Pharmacokinetics
  • Absorption: Minimal systemic absorption via the ocular route
  • Distribution: Localized primarily to the tear film and ocular surface
  • Metabolism: Not systemically metabolized
  • Excretion: Eliminated via natural tear drainage
  • Onset: Immediate symptomatic relief upon instillation
  • Duration: Effects last several hours depending on tear film stability and severity of dryness
Pregnancy Category & Lactation
  • Pregnancy: Considered Category C (no controlled human studies); topical ocular use has negligible systemic absorption, making risk minimal
  • Breastfeeding: Safe for use during lactation; minimal systemic absorption implies negligible exposure to the infant
  • Caution: As with all ocular products, consult a healthcare provider if used frequently
Therapeutic Class
  • Primary Class: Ophthalmic lubricant / Artificial tear
  • Subclass: Viscosity-enhancing polymer with humectant
Contraindications
  • Known hypersensitivity to hydroxyethyl cellulose, glycerin, or any excipients
  • Eye infection or corneal ulcer unless under physician supervision
Warnings & Precautions
  • Avoid contamination of the dropper tip to prevent secondary eye infections
  • Do not use if solution changes color or becomes cloudy
  • Discontinue use if eye pain, redness, or visual disturbance occurs and seek medical advice
  • High-risk patients: immunocompromised individuals, post-surgical eyes
Side Effects
  • Common: Temporary mild stinging or burning upon instillation
  • Ocular: Blurred vision immediately after administration (resolves within minutes)
  • Rare: Allergic conjunctivitis or eyelid edema
  • Timing: Usually occurs immediately and resolves quickly; no dose-dependence noted
Drug Interactions
  • Minimal systemic absorption means negligible drug-drug interactions
  • Can be used concomitantly with other topical ophthalmic medications; administer at least 5–10 minutes apart to avoid dilution
  • No significant interactions with food or alcohol
Recent Updates or Guidelines
  • Guideline Recommendations: Artificial tears with hydroxyethyl cellulose and glycerin are recommended as first-line management for dry eye syndrome in TFOS DEWS II guidelines
  • Emphasis on preservative-free formulations for frequent use to minimize ocular surface toxicity
  • No changes in approved indications or dosing in recent years
Storage Conditions
  • Store at 20°C to 25°C (room temperature)
  • Protect from light, moisture, and freezing
  • Keep the bottle tightly closed when not in use
  • Avoid contamination of the dropper tip
  • Discard after 30 days of opening to prevent microbial growth
