Indications

Approved Indications

  • Topical symptomatic relief of musculoskeletal pain, including:
    • Minor muscle or joint pain (e.g., due to backache, sprains, strains)
    • Arthritis-related pain (osteoarthritis, rheumatoid arthritis – mild/moderate)
    • Simple neuralgia
    • Sports injuries
    • Tendonitis and bursitis (non-infectious)
    • Stiffness, cramps, or muscle fatigue
  • Topical counterirritant therapy to provide a cooling or warming sensation and distract from deeper pain

Important Off-label (Clinically Accepted) Uses

  • Myofascial pain syndrome (local application over trigger points)
  • Minor post-herpetic neuralgia (adjunctive use)
  • Pain from fibromyalgia (as part of multimodal pain management)
Dosage & Administration

Route of Administration: Topical only (external use)

Adults and Adolescents (≥12 years)

  • Apply a thin layer of cream/gel/ointment to the affected area 3 to 4 times daily
  • Massage gently into the skin until fully absorbed
  • Transdermal patches: Apply one patch to the painful area; change every 8–12 hours
  • Spray or roll-on: Apply directly without rubbing if directed; avoid overuse
  • Duration: Use for up to 7 days unless otherwise instructed by a physician

Pediatrics (<12 years)

  • Not recommended without medical advice due to risk of salicylate toxicity or skin irritation

Elderly

  • Use same adult dosing; monitor for increased skin sensitivity

Special Populations

Mechanism of Action (MOA)

Methyl Salicylate acts as a topical analgesic and anti-inflammatory agent by inhibiting cyclooxygenase (COX) enzymes, reducing local prostaglandin synthesis, and thus producing anti-inflammatory and analgesic effects. Menthol functions as a counterirritant by activating TRPM8 receptors on the skin, which creates a cooling sensation that distracts the nervous system from underlying pain signals. The combination enhances perceived relief by simultaneously dampening nociceptor sensitivity and stimulating superficial nerve endings.
