• Nasal spray:

• Nasal spray:
• Adults and children ≥6 years: 2–3 sprays per nostril every 10–12 hours as needed
• Children 2–6 years: Use under medical supervision, typically 1 spray per nostril twice daily
• Do not exceed 3 consecutive days of use to prevent rebound congestion
• Ophthalmic drops:
• Instill 1–2 drops into the affected eye(s) up to 4 times daily as needed
• Special populations:
• Elderly: Use standard dosing with monitoring for cardiovascular effects
• Renal or hepatic impairment: No dose adjustment needed

Oxymetazoline hydrochloride is a direct-acting sympathomimetic agent that selectively stimulates alpha-1 adrenergic receptors in the vascular smooth muscle. Activation of these receptors causes vasoconstriction of blood vessels in the nasal mucosa and conjunctiva, leading to decreased blood flow, reduced swelling, and relief of nasal congestion and ocular redness.

• Absorption: Minimal systemic absorption after topical nasal or ocular administration
• Distribution: Primarily localized to application site; systemic distribution negligible
• Metabolism: Minor hepatic metabolism, not clinically significant
• Elimination: Excreted primarily unchanged via the kidneys
• Onset of action: 5–10 minutes
• Duration of effect: 6–10 hours

• Pregnancy: Category C – Animal studies indicate risk at high doses; use only if benefits justify potential risks
• Lactation: Minimal systemic absorption; caution advised when used during breastfeeding

• Nasal decongestant and topical vasoconstrictor
• Alpha-adrenergic receptor agonist (sympathomimetic)

• Hypersensitivity to oxymetazoline or formulation components
• Use with monoamine oxidase inhibitors (MAOIs) or within 14 days of MAOI therapy
• Severe hypertension or cardiovascular disease (relative contraindication)
• Narrow-angle glaucoma (ophthalmic form)

• Limit nasal use to 3 consecutive days to avoid rhinitis medicamentosa (rebound congestion)
• Use with caution in patients with hypertension, ischemic heart disease, or arrhythmias
• Avoid prolonged use in eyes to prevent increased intraocular pressure
• Prevent contamination of dropper tip to reduce infection risk
• Supervise use in children under 6 years

• Common: Nasal dryness, irritation, sneezing, burning sensation, headache
• Less common: Increased blood pressure, palpitations, nervousness
• Rare: Rebound nasal congestion, allergic reactions, ocular hypertension (ophthalmic use)

• Contraindicated with MAO inhibitors due to risk of hypertensive crisis
• Additive effects with other sympathomimetics may increase blood pressure
• Possible interaction with tricyclic antidepressants and beta-blockers
• No significant CYP450 interactions due to minimal systemic absorption

• Emphasized restriction of nasal spray use to ≤3 days to prevent rebound congestion
• No recent major changes in approved indications or warnings by FDA or EMA
• Ongoing recommendation for cardiovascular monitoring in at-risk patients

• Store at 20°C to 25°C (68°F to 77°F)
• Protect from moisture and direct sunlight
• Keep container tightly closed

• Discard after expiration or if product appearance changes
