Approved Indications

• Biotin deficiency (rare but serious)
• Biotin-responsive inherited metabolic disorders, including:
• Biotinidase deficiency
• Holocarboxylase synthetase deficiency
• Supportive treatment for peripheral neuropathy (adjunct, based on clinical use)

Clinically Accepted / Off-Label Uses

• Improving hair thinning, brittle nails, and dry skin in suspected deficiency
• Supportive use in pregnancy to correct a marginal deficiency (common but usually mild)
• Adjunct in diabetes management for modest improvement in glucose control (limited evidence)

Adults

• Biotin deficiency: 5 mg to 10 mg daily (severe cases may require up to 20 mg/day)
• Biotinidase deficiency: 5 mg to 20 mg/day
• General supplementation: 30–100 mcg/day

Pediatric

• Infants: 5–10 mcg/day (supplement)
• Children: 10–20 mcg/day
• Biotinidase deficiency: 5–10 mg/day

Elderly

• No dosage adjustment required.

Pregnancy & Lactation

• Pregnancy: 30 mcg/day (RDA)
• Lactation: 35 mcg/day

Administration

• Taken orally, with or without food.
• High-dose biotin should be taken separately from multivitamins to avoid assay interference.

Biotin acts as a coenzyme for carboxylase enzymes, essential for:

• Fatty acid synthesis
• Amino acid metabolism
• Gluconeogenesis
• Energy production (ATP generation)

It supports keratin infrastructure, leading to improved hair and nail strength.

• Absorption: Well absorbed from the small intestine.
• Distribution: Widely distributed; accumulates in liver, brain, and muscle.
• Metabolism: Minimal; excess circulates unmetabolized.
• Excretion: Primarily excreted unchanged in urine.
• Half-life: 26–36 hours (varies with dose).

• Pregnancy: Generally safe at recommended doses; mild deficiency is common during pregnancy.
• Lactation: Safe; biotin is naturally present in breast milk.

High-dose biotin (5–20 mg/day) should be used only under medical supervision.

Water-soluble B-complex vitamin (Vitamin B7).
Coenzyme / Nutritional supplement.

• Known hypersensitivity to biotin
• Avoid very high doses in patients undergoing frequent lab testing (interferes with immunoassays)

• High-dose biotin (≥5 mg/day) can interfere with laboratory tests, including:
• Thyroid function tests
• Troponin (cardiac) assays
• Hormone levels
• Vitamin levels
• Misleading test results may lead to misdiagnosis.

Stop biotin 2–3 days before lab tests.

Common (usually mild):

• Nausea
• Stomach upset
• Skin rash

Rare:

• Acne-like rash
• Allergic reactions

• Anticonvulsants (carbamazepine, phenytoin, phenobarbital) – decrease biotin levels
• Long-term antibiotics – reduce gut flora and lower biotin absorption
• Alpha-lipoic acid – may reduce biotin effectiveness
• Lab test interference – very significant at high doses

• FDA warns that high-dose biotin severely interferes with lab tests, including troponin, thyroid tests, and immunoassays.
• NIH notes increasing use for hair and nail health, though strong evidence exists only for deficiency-related symptoms.
