Indications
  • Wound Cleansing and Disinfection: Used for cleansing and disinfecting minor cuts, abrasions, burns, ulcers, and infected wounds to prevent and control infection.
  • Preoperative Skin Preparation: Applied before minor surgical or invasive procedures to reduce microbial flora on skin.
  • Management of Local Skin Infections: For topical treatment of superficial bacterial infections and to aid healing.
  • Mouthwash and Oral Hygiene (in some formulations): Used to reduce oral bacterial load in gingivitis and stomatitis (formulation dependent).
  • Off-label Uses: Sometimes used in diabetic foot ulcers and pressure sores under medical supervision.
Dosage & Administration
  • Topical Application: Clean the affected area gently, then apply a thin layer of the solution, cream, or ointment containing Chlorhexidine Gluconate + Cetrimide.
  • Frequency: Usually applied 1-3 times daily depending on wound severity and clinical judgment.
  • Duration: Continue treatment until signs of infection resolve or as directed by a healthcare professional, typically not exceeding 7-14 days without reassessment.
  • Special Populations: Use cautiously in children and elderly; no specific dose adjustment is required.
  • Avoid: Application to deep or puncture wounds, large surface areas, or mucous membranes unless specifically formulated for such use.
Mechanism of Action (MOA)

Chlorhexidine gluconate is a cationic bisbiguanide antiseptic that binds to negatively charged bacterial cell membranes, disrupting their integrity and causing leakage of intracellular contents, resulting in bactericidal effects. Cetrimide, a quaternary ammonium compound, acts as a surfactant and antiseptic by disrupting microbial cell membranes and causing protein denaturation. Together, these agents provide a broad-spectrum antimicrobial action against Gram-positive and Gram-negative bacteria, fungi, and some viruses. Their combined action enhances microbial cell wall disruption, improving efficacy in wound disinfection and prevention of infection.

Pharmacokinetics
  • Absorption: Minimal systemic absorption through intact skin. Absorption may increase if applied to large, damaged, or burned skin areas.
  • Distribution: Mainly remains localized at the site of application.
  • Metabolism: Not systemically metabolized due to minimal absorption.
  • Excretion: Not applicable for topical formulations with negligible systemic absorption.
  • Half-life: Not clinically relevant for topical use.
Pregnancy Category & Lactation
  • Pregnancy: No adequate controlled studies in pregnant women; topical use considered low risk due to minimal systemic absorption but should be used only if clearly needed and benefits outweigh risks.
  • Lactation: Limited data available; considered safe for topical use with negligible systemic absorption. Caution advised when applied to large areas.
Therapeutic Class
  • Topical antiseptic combination.
Contraindications
  • Known hypersensitivity to chlorhexidine, cetrimide, or any formulation excipients.
  • Application to deep or puncture wounds, extensive burns, or large open wounds without medical supervision.
  • Not for ophthalmic, otic, or mucosal use unless specifically formulated.
  • Severe skin conditions or dermatitis at application site.
Warnings & Precautions
  • Avoid contact with eyes, ears, and mucous membranes.
  • Discontinue if hypersensitivity or severe local irritation occurs.
  • Rare anaphylactic reactions have been reported; monitor patients with known allergies.
  • Use cautiously in children and elderly due to potential for increased skin sensitivity.
  • Avoid prolonged use on large skin areas to prevent systemic toxicity risk.
  • Not a substitute for systemic antibiotic therapy in serious infections.
Side Effects

Common:

  • Local irritation, burning, itching, or redness at the application site.
  • Mild skin dryness or scaling.

Serious/Rare:

  • Allergic contact dermatitis.
  • Anaphylaxis (very rare).
  • Skin sensitization and hypersensitivity reactions.
Drug Interactions
  • No significant systemic drug interactions expected due to negligible systemic absorption.
  • May be inactivated by soaps, detergents, or other topical products; avoid concurrent use.
  • No known food or alcohol interactions.
Recent Updates or Guidelines
  • Continued recommendation for combined use in topical wound antisepsis due to synergistic antimicrobial effects.
  • Increased clinical caution about allergic reactions has been emphasized in recent safety communications.
  • No major changes in dosing or indications reported by regulatory bodies.
Storage Conditions
  • Store at 20°C to 30°C (68°F to 86°F).
  • Protect from freezing, excessive heat, and direct sunlight.
  • Keep container tightly closed and avoid contamination.
  • Use within expiry period; discard unused product if discolored or contaminated.
