Indications

Approved Indications:

  • Wound Healing: Treatment and support of healing in minor wounds, cuts, abrasions, burns, and skin ulcers.
  • Skin and Mucous Membrane Disorders: Used for dryness, irritation, and inflammation of the skin and mucous membranes.
  • Topical Use in Dermatology: For conditions such as diaper rash, eczema, and dermatitis.
  • Postoperative Care: To promote healing of surgical wounds.
  • Nasal and Ear Mucosa: Moisturizing and promoting regeneration in conditions like nasal dryness or after nasal surgery.
  • Hair and Scalp Care: Used to improve scalp health and support hair growth in some formulations.

Off-label or Clinically Accepted Uses:

  • Supportive therapy in radiation dermatitis.
  • Treatment of mouth ulcers and stomatitis.
  • Use in combination with other agents for treatment of chronic wounds and ulcers.
Dosage & Administration

Adults and Pediatrics:

  • Topical Application: Apply a thin layer of ointment, cream, or lotion to the affected area 2–3 times daily.
  • Nasal Drops/Spray: Instill as per product instructions, typically 1–2 drops or sprays per nostril 2–3 times daily.
  • Eye Drops (in some formulations): Administer as prescribed, usually 1–2 drops several times daily.
  • Parenteral (Intramuscular/Intravenous) Use: Administer doses vary depending on clinical need; commonly 250–1000 mg per day divided into 2–3 doses, under medical supervision.

Special Populations:

  • Elderly: No specific dosage adjustments required; monitor for skin sensitivity.
  • Renal/Hepatic Impairment: No dose adjustments necessary.
  • Pregnant and Lactating Women: Topical use considered safe; systemic use only if clearly indicated.
Mechanism of Action (MOA)

Dexpanthenol is the alcohol analog of pantothenic acid (vitamin B5) and serves as a provitamin that is rapidly converted in the body to pantothenic acid. Pantothenic acid is an essential component of coenzyme A, which plays a critical role in cellular metabolism, including the synthesis and repair of fatty acids, lipids, and proteins. By enhancing cellular energy metabolism and promoting epithelial regeneration, dexpantenol accelerates wound healing, reduces inflammation, and improves skin hydration and barrier function.

Pharmacokinetics
  • Absorption: Rapidly absorbed and converted to pantothenic acid after topical or parenteral administration.
  • Distribution: Pantothenic acid is distributed throughout the body tissues, including skin and mucous membranes.
  • Metabolism: Metabolized intracellularly to coenzyme A, which participates in numerous metabolic pathways.
  • Half-life: Short plasma half-life of pantothenic acid (about 1 hour), but biological effects persist due to involvement in coenzyme A function.
  • Excretion: Excreted primarily in urine as metabolites of pantothenic acid.
Pregnancy Category & Lactation
  • Pregnancy: No known teratogenic or embryotoxic effects; topical use is considered safe. Systemic use should be limited to cases where benefits outweigh risks.
  • Lactation: Topical use is safe. Limited data on systemic use during breastfeeding; caution advised.
