Indications
  • Approved Uses:
    • Prevention and treatment of allergic conjunctivitis (seasonal and perennial).
    • Management of vernal keratoconjunctivitis.
    • Relief of ocular itching, redness, and inflammation caused by allergens.
  • Off-label Uses:
    • Prophylaxis of ocular allergy symptoms during allergen exposure.
    • Adjunctive treatment in other allergic eye conditions as determined by a specialist.
Dosage & Administration
  • Adults and Children (≥4 years):
    • Instill 1 drop of 2% sodium cromoglicate into the affected eye(s) 4 times daily.
    • During severe symptoms, dosing may increase to every 2 hours (up to 6–8 times daily).
    • Begin treatment before allergen exposure for prophylaxis.
  • Elderly:
    • No dose adjustment required.
  • Special Populations (Renal/Hepatic Impairment):
    • No adjustments needed due to minimal systemic absorption.
  • Route: Topical ocular administration.
  • Duration: Continue as long as symptoms persist or throughout allergen exposure.
Mechanism of Action (MOA)

Sodium cromoglicate stabilizes mast cells by preventing calcium influx necessary for degranulation. This inhibits the release of histamine and other inflammatory mediators responsible for allergic symptoms. By stabilizing mast cells in conjunctival tissues, it reduces ocular itching, redness, and swelling associated with allergic conjunctivitis.

Pharmacokinetics
  • Absorption: Poorly absorbed systemically after ocular administration; remains mainly on the ocular surface.
  • Distribution: Limited to ocular tissues.
  • Metabolism: Not metabolized significantly due to minimal systemic exposure.
  • Excretion: Any absorbed portion is excreted unchanged renally.
  • Onset: Symptom improvement generally occurs after several days of regular use.
  • Half-life: Systemic half-life negligible due to minimal absorption.
Pregnancy Category & Lactation
  • Pregnancy: FDA category B — animal studies show no risk; human data insufficient. Use only if clearly needed.
  • Lactation: Excretion in breast milk unknown; systemic absorption minimal. Use with caution.

Contraindications
  • Hypersensitivity to sodium cromoglicate or any formulation component.
  • Presence of active ocular infections or severe ocular surface disease.
Warnings & Precautions

  • Discontinue if severe irritation or hypersensitivity occurs.
  • Not effective for immediate relief of acute allergic symptoms.
  • Use cautiously in patients with dry eye syndrome or corneal damage.
Side Effects
  • Common: Mild transient eye irritation, burning, stinging, or blurred vision immediately post-instillation.
  • Rare: Hypersensitivity reactions such as conjunctival hyperemia, swelling, or itching.
Drug Interactions
  • No clinically significant interactions due to minimal systemic absorption.
  • Can be safely combined with antihistamine eye drops or lubricants.
