Indications
  • Approved Indications:
    • Treatment of bacterial vaginosis (as a local antiseptic agent).
    • Treatment of vaginal infections caused by susceptible organisms, including bacterial and fungal infections.
    • Oral antiseptic in throat infections and oral mucosal infections (lozenge or mouthwash formulations).
    • Used in oral candidiasis and oral mucositis management.
    • Adjunctive treatment in vaginal flora imbalance.
  • Clinically Accepted Off-label Uses:
    • Prophylaxis for infections in minor surgical procedures of the oral cavity or vaginal area.
    • Treatment of localized infections in the upper respiratory tract.
    • Sometimes used in formulations for wound antisepsis.
Dosage & Administration
  • Vaginal Use:
    • Tablet insertion: One vaginal tablet containing 10 mg dequalinium chloride inserted once daily at bedtime for 6 days.
    • Alternative dosing may vary based on formulation and product instructions.
    • Not recommended for prolonged continuous use beyond prescribed duration.
  • Oral Use:
    • Lozenges: Typically one lozenge dissolved slowly in the mouth 1 to 3 times daily for 7 to 14 days, depending on severity.
    • Mouthwash: Diluted as per product instructions, gargled or used to rinse oral cavity 2-3 times daily.
  • Pediatrics:
    • Safety and efficacy in children not well established; use with caution.
  • Elderly:
    • No specific dosage adjustment required.
  • Special Populations:
    • No dosage adjustment necessary for renal or hepatic impairment due to topical/local administration and minimal systemic absorption.
Mechanism of Action (MOA)

Dequalinium chloride is a quaternary ammonium compound with broad-spectrum antimicrobial activity. It exerts its therapeutic effect by disrupting microbial cell membranes due to its cationic surfactant properties. This results in increased permeability, leakage of intracellular components, and ultimately bacterial and fungal cell death. It is effective against Gram-positive and Gram-negative bacteria as well as yeasts, making it suitable as a topical antiseptic and antimicrobial agent in mucosal infections.

Pharmacokinetics
  • Absorption:
    • Minimal systemic absorption following vaginal or oral topical use.
  • Distribution:
    • Limited to site of application; systemic distribution negligible.
  • Metabolism:
    • Not significantly metabolized due to minimal absorption.
  • Half-life:
    • Not applicable given local action.
  • Elimination:
    • Excreted mainly via local mucosal clearance; systemic elimination data not relevant due to minimal absorption.
Pregnancy Category & Lactation
  • Pregnancy:
    • No adequate controlled studies in pregnant women.
    • Use only if clearly needed; local application limits systemic exposure.
    • Considered relatively safe when used topically in recommended doses.
  • Lactation:
    • No data on excretion into breast milk.
    • Caution advised; local use unlikely to cause significant infant exposure.
Therapeutic Class
  • Primary Therapeutic Class: Topical Antiseptic and Antimicrobial Agent
  • Subclass: Quaternary ammonium compound antiseptic
Contraindications
  • Known hypersensitivity or allergy to dequalinium chloride or formulation excipients.
  • Presence of open wounds or severe mucosal damage where local irritation is a risk.
  • Not recommended in cases of deep-seated infections requiring systemic therapy.
Warnings & Precautions
  • Avoid use in patients with known allergy to quaternary ammonium compounds.
  • Discontinue if signs of local irritation, burning, or allergic reaction occur.
  • Not a substitute for systemic antimicrobial therapy when indicated.
  • Use with caution in pregnant or breastfeeding women unless benefit outweighs risk.
  • Not for use in neonates or infants unless specifically indicated.
  • Prolonged or excessive use may disrupt normal mucosal flora.
Side Effects
  • Common:
    • Local irritation, burning sensation, itching, or redness at application site.
    • Mild gastrointestinal upset if oral lozenges swallowed rapidly.
  • Rare/Serious:
    • Allergic reactions including rash or contact dermatitis.
    • Hypersensitivity reactions such as swelling or difficulty breathing (very rare).
  • Side effects are generally mild and resolve after discontinuation.
Drug Interactions
  • Minimal systemic absorption results in low potential for systemic drug interactions.
  • No known significant interactions with other topical or systemic agents.
  • Concurrent use with other local antiseptics may increase mucosal irritation.
Recent Updates or Guidelines
  • Clinical guidelines reinforce use in bacterial vaginosis and local oral infections as a safe and effective topical antiseptic.
  • Emphasis on short-term use to prevent mucosal flora disruption.
  • No recent changes in dosing recommendations.
  • Studies continue to evaluate broader antiseptic applications and resistance profiles.
Storage Conditions
  • Store at 20°C to 25°C (68°F to 77°F).
  • Protect from moisture and light.
  • Keep in original packaging until use.
  • Do not freeze.
