Indications

Approved Indications:

  • Ophthalmic Use: Treatment and prevention of dry eyes and irritation.
  • Eye Surgery Adjunct: Used as a protective lubricant and volume expander during cataract surgery, corneal transplantation, and other ocular surgeries.
  • Artificial Tear Substitute: For symptomatic relief of ocular dryness caused by environmental factors, contact lens wear, or insufficient tear production.
  • Corneal Protection: To protect the corneal epithelium from damage due to surgical trauma, laser procedures, or chemical exposure.

Off-label or Clinically Accepted Uses:

  • Postoperative eye care to promote healing and comfort.
  • Management of mild to moderate keratoconjunctivitis sicca.
  • Supportive treatment for ocular surface disorders.
Dosage & Administration
  • Route: Topical ophthalmic solution (eye drops).
  • Adults and Pediatrics:
    • Instill 1-2 drops in the affected eye(s) as needed, typically 3 to 6 times daily.
    • Frequency may be increased for symptomatic relief, but usually not exceeding 12 drops/day.
  • Elderly: No dosage adjustment necessary; same regimen as adults.
  • Special Populations: No specific adjustments needed for renal or hepatic impairment due to topical route and minimal systemic absorption.
  • Administration Instructions:
    • Wash hands before use.
    • Avoid contact of dropper tip with eye or any surface to prevent contamination.
    • Close eyes gently after instillation and do not blink excessively.
    • Wait at least 5 minutes between administration of different eye drops.
Mechanism of Action (MOA)

Dextran is a high molecular weight polysaccharide that acts as a lubricant and volume expander in the tear film, enhancing ocular surface hydration and protecting the corneal epithelium from mechanical and desiccation stress. Hypromellose (hydroxypropyl methylcellulose) is a viscous cellulose derivative that increases tear film viscosity and stabilizes the ocular surface by retaining moisture, reducing evaporation, and providing sustained lubrication. Together, these agents mimic natural tears and promote corneal epithelial healing and comfort.

Pharmacokinetics
  • Absorption: Minimal systemic absorption following topical ocular administration.
  • Distribution: Primarily remains on the ocular surface; forms a protective film over the cornea and conjunctiva.
  • Metabolism: Not systemically metabolized; locally degraded and cleared by tear turnover.
  • Elimination: Removed from the ocular surface by normal tear drainage and blinking.
  • Onset of Action: Immediate symptomatic relief upon instillation.
