Indications
  • Scabies: Treatment of infestation by Sarcoptes scabiei (human itch mite), including classic scabies and Norwegian (crusted) scabies.
  • Pediculosis (Lice): Effective for head lice (Pediculus humanus capitis) and pubic lice (Pthirus pubis) infestations.
  • Off-label uses:
    • Prevention and treatment of lice infestation in institutional outbreaks.
    • Occasionally used for other ectoparasitic infestations under medical supervision.

Dosage & Administration
  • Route: Topical application only.
  • For Scabies:
    • Apply 5% permethrin cream over the entire body from the neck down, including under nails and skin folds.
    • Leave on for 8–14 hours (usually overnight), then wash off.
    • Repeat treatment in 7 days if needed.
  • For Head Lice:
    • Apply 1% permethrin lotion to dry hair and scalp, thoroughly covering all hair.
    • Leave for 10 minutes, then rinse off with water.
    • Repeat treatment after 7–10 days if live lice are still present.
  • Pediatrics:
    • Safe for children over 2 months old for scabies; for lice, used in children over 2 years old.
    • Dosage same as adults, adjusted by hair length for lice treatment.
  • Elderly: No specific dosage adjustment but monitor skin integrity.
  • Special populations: Use with caution in pregnant or breastfeeding women; consult healthcare provider.

Mechanism of Action (MOA)

Permethrin is a synthetic pyrethroid insecticide that targets the nervous system of parasites. It acts by disrupting sodium ion channels on neuronal membranes, leading to prolonged depolarization and paralysis of the mites or lice. This results in paralysis and death of the parasites, effectively eradicating the infestation. Its selective toxicity is due to greater sensitivity of insect nerve cells compared to mammals.

Pharmacokinetics
  • Absorption: Minimal systemic absorption through intact skin; some absorption may occur if skin is damaged or inflamed.
  • Distribution: Due to low systemic absorption, plasma levels are typically negligible.
  • Metabolism: If absorbed, metabolized rapidly in the liver by ester hydrolysis and oxidation.
  • Elimination: Mainly excreted via urine and feces as inactive metabolites.
  • Onset: Parasite killing occurs within minutes to hours after topical application.
Pregnancy Category & Lactation
  • Pregnancy: Category B (FDA) – Animal studies have not shown risk to the fetus; however, no adequate well-controlled studies in pregnant women. Use only if clearly needed.
  • Lactation: Permethrin is minimally absorbed and considered compatible with breastfeeding when used topically; avoid application on nipples to prevent infant exposure.

Contraindications
  • Known hypersensitivity or allergy to permethrin or any formulation excipients.
