Indications
  • Approved Indications:
    • Treatment of intestinal nematode infections, including:
      • Ascariasis (caused by Ascaris lumbricoides)
      • Enterobiasis (Pinworm infection) (caused by Enterobius vermicularis)
      • Trichostrongyliasis and other hookworm infections (e.g., Ancylostoma duodenale, Necator americanus)
    • Effective against other intestinal roundworms sensitive to pyrantel.
  • Off-label / Clinically Accepted Uses:
    • Occasionally used for prophylaxis in endemic areas.
    • Sometimes prescribed for mixed helminth infections where multiple intestinal nematodes are present.
Dosage & Administration
  • Adults and Children over 2 years:
    • Single dose of 11 mg/kg (maximum 1 gram per dose) orally.
    • Dose may be repeated after 2 weeks if reinfection or persistent infection is suspected.
  • Children under 2 years:
    • Use only if benefits outweigh risks; pediatric formulations and dosing should be guided by a physician.
  • Administration:
    • Oral tablets or suspension.
    • Can be taken with or without food.
    • For best effect, whole dose should be taken at once.
  • Special Populations:
    • No specific dosage adjustment for renal or hepatic impairment documented, but caution advised in severe cases.
    • Elderly: Use standard dosing; monitor for tolerance.
Mechanism of Action (MOA)

Pyrantel pamoate is a depolarizing neuromuscular blocking agent that selectively stimulates nicotinic acetylcholine receptors on nematode muscle cells, causing persistent activation leading to spastic paralysis. The paralyzed worms lose their grip on the intestinal mucosa and are expelled by normal peristalsis, thereby eradicating the infection.

Pharmacokinetics
  • Absorption: Minimal systemic absorption after oral administration; most remains in the gastrointestinal tract.
  • Distribution: Limited systemic distribution due to poor absorption.
  • Metabolism: Negligible systemic metabolism; primarily acts locally in the gut.
  • Excretion: Mainly eliminated unchanged in feces; minimal renal excretion.
  • Onset of action: Rapid, within hours of administration.
Pregnancy Category & Lactation
  • Pregnancy: Classified as FDA Pregnancy Category C. Animal studies have not demonstrated teratogenic effects at therapeutic doses, but human data are limited; use only if clearly needed.
  • Lactation: Pyrantel is excreted in breast milk in minimal amounts; generally considered compatible with breastfeeding, but caution advised.

Contraindications
