Oral Powder
ACI Limited

Each sachet: ৳ 7.00 (20's pack: ৳ 140.00)

Indications
  • Acute Diarrhea: Treatment and prevention of dehydration caused by acute diarrhea in infants, children, and adults.
  • Cholera: Adjunct therapy for rehydration in cholera patients.
  • Dehydration: Due to vomiting, excessive sweating, heat exposure, or other causes of fluid loss.
  • Maintenance of Hydration: For mild to moderate dehydration requiring electrolyte and fluid replacement.
  • Off-label Uses: Supportive care in gastroenteritis and during physical exertion with fluid loss.
Dosage & Administration
  • Route: Oral, reconstituted solution.
  • Preparation: Dissolve one sachet (approximately 20 grams) in 1 liter of clean, potable water.
  • Adults: Replace fluid loss as needed; typically 2–3 liters per day or as clinically indicated.
  • Children: Follow weight-based dosing: approximately 50–100 mL/kg over 3–4 hours for rehydration, then maintenance fluids as required.
  • Elderly: Dose according to hydration status; no specific adjustment.
  • Special Populations:
    • Renal impairment: Use cautiously; monitor electrolytes closely.
    • Hepatic impairment: No specific dose adjustments required.
  • Frequency: Administer repeatedly to replace ongoing fluid losses until hydration is restored.
Mechanism of Action (MOA)

ORS utilizes the sodium-glucose co-transport mechanism in the intestinal mucosa to enhance the absorption of sodium and water. Glucose facilitates sodium uptake, which promotes passive water absorption, thereby rapidly restoring fluid and electrolyte balance. Flavored formulations improve taste and patient compliance, encouraging adequate oral intake.

Pharmacokinetics
  • Absorption: Glucose and electrolytes are rapidly absorbed from the small intestine.
  • Distribution: Electrolytes distribute mainly in the extracellular fluid compartment.
  • Metabolism: Glucose undergoes normal cellular metabolism.
  • Excretion: Excess electrolytes and water are excreted primarily via renal pathways.
Pregnancy Category & Lactation
  • Pregnancy: Safe for use; no known adverse effects on the fetus. Essential for preventing dehydration.
  • Lactation: Considered safe; supports maternal hydration and milk production.
Therapeutic Class
  • Electrolyte and fluid replacement therapy; oral rehydration solution.
Contraindications
  • Hypersensitivity to any component of the formulation (rare).
  • Severe dehydration requiring intravenous therapy.
  • Conditions such as intestinal obstruction or inability to tolerate oral fluids.
Warnings & Precautions
  • Monitor for worsening dehydration or electrolyte imbalance.
  • In case of persistent vomiting or inability to retain fluids, intravenous rehydration is necessary.
  • Use with caution in renal impairment.
  • Not adequate as sole treatment in severe dehydration or shock.
Side Effects
  • Common: Minimal; may include mild gastrointestinal discomfort or bloating.
  • Rare: Electrolyte imbalance or hypernatremia if misused or over-administered.
  • No serious adverse effects when used appropriately.
Drug Interactions
  • No known significant drug interactions.
  • Oral medications should not be mixed with ORS to avoid absorption interference; separate administration times are advised.
Recent Updates or Guidelines
  • WHO recommends flavored ORS formulations to enhance acceptability and compliance.
  • Zinc supplementation is encouraged alongside ORS in pediatric diarrhea management.
  • ORS remains first-line therapy for dehydration due to diarrhea globally.
Storage Conditions
  • Store dry powder sachets at 20°C to 30°C, protected from moisture, heat, and direct sunlight.
  • Use reconstituted solution within 24 hours; refrigerate if desired and discard unused portion thereafter.
  • Do not freeze.
